Must be referred by hospital or medical clinic. Transitional and long term care for individuals patients who require medical and physical rehabilitation after a hospital stay specializing in clinical services such as physician oversight nurses physical occupational and speech therapies social workers and licensed nutritionists. Long-term care can often be the best option for individuals who require around-the-clock nursing care and clinical services.
